Since we understand that unexpected injuries are difficult to plan for financially, we will work with you to find an acceptable payment plan.

If your insurance plan requires a referral, please obtain one from your primary care physician prior to your visit. Patients are responsible for their co-payments and deductibles at the time services are rendered. We are a very transparent office; there are no hidden costs in a facility fee. You are always free to contact our office for additional information about your specific insurance plan.
